DPH releases COVID-19 Vaccine Pre-Enrollment Questionnaire, enrollment required for healthcare providers intending to provide vaccines to their staff, patients, residents upon availability

December 7, 2020- The Georgia Department of Public Health (DPH) releases a COVID-19 Vaccine Pre-Enrollment Questionnaire for healthcare provider intending to provide COVID-19 vaccines to their staff, patients, or residents upon vaccine availability. Pre-enrollment is the first step in the process to become a vaccine provider. Failing to pre-enroll will result in vaccines not being allocated for your you.

A few important points:
- This survey will allow the Georgia Immunization Program to assess your current capabilities, needs etc.
- Even if you are already registered to provide other vaccines- you must register separately for the COVID-19 Vaccine
- Entities do not have to be a validated Closed POD to be a provider. However, being a Closed POD helps with necessary planning to vaccinate your population(s) and ensures current contact information for the facility
- Entities do not have to be in the Tier 1 group to start enrolling
- If you are a Long Term Care Facility, and have already signed on with a pharmacy (CVS or Walgreens) to provide your COVID-19 Vaccine, you do not need to complete the pre-enrollment survey!
