Invitation to provide input to the 2021 Georgia Primary Care Access Plan

December 11, 2020- The Georgia Primary Care Office at the Georgia Department of Community Health’s State Office of Rural Health is crafting the Georgia Primary Care Access Plan for 2021.  The Plan will illustrate current needs of Georgia's underserved people and communities.  It will serve as a resource guide to help target investments and interventions to improve the health underserved Georgians across the State.

To ensure that the Georgia Primary Care Access Plan illustrates the current state of access for primary, mental and dental health services that are reflective of your community we are asking you to complete a short survey that most will complete within 10 minutes.
